Preparing for Your First Work Injury Visit
By Work Comp Chicago Editorial Staff · Published · Last reviewed
What to bring
- A photo ID
- Your employer's name and address
- The date of the injury and a short description of what happened
- Any claim number or insurer information you have been given
- A list of any medications you take and any prior injuries to the same area
What to be ready to describe
What you were doing when it happened, how it happened, what you felt right away and what you feel now, and what your job actually requires: weights lifted, hours standing, repeated motions, driving time. That detail shapes both the treatment plan and the work status note.
What happens at the visit
- A history of the incident and your job duties
- A physical examination of the injured area
- A treatment plan written for your injury and the work you do
- A work status note describing what you can and cannot do while you recover
- Referrals for imaging or pain management when the provider finds they are needed
What to do with the work status note
Give it to your employer, keep a copy, and tell the provider if the work you are given goes beyond the restrictions. It is updated at follow-up visits as you improve.
Common questions
Do I pay at the visit?+
For a covered work injury, the provider bills the employer or its insurer directly. Staff can explain how billing applies to your visit before you are seen.
How long is the first visit?+
Long enough for a full history, an examination and a treatment plan. Staff will give you a sense of timing when they confirm your appointment.
Can I bring someone with me?+
Yes. A family member or friend can come with you to the visit.
